Frequently Asked Questions
General Practice Information
Do you accept insurance?
We are an out-of-network practice and do not accept insurance. This allows us to offer a more personalized, confidential, and high-quality clinical experience, without the constraints of insurance company policies. Many clients choose to use FSA or HSA cards, which we gladly accept.
Where are appointments held?
We offer both in-person and virtual appointments. In-person visits take place in a welcoming, comfortable office space in Richmond, Virginia. Telehealth is available for clients who prefer the convenience or flexibility of virtual care.
How do I pay for sessions?
We require a credit card, debit card, or FSA/HSA card on file to reserve your appointment. Payment is processed securely, and we can provide a ‘superbill’ for you to submit to insurance for possible reimbursement.
Do you complete disability or FMLA paperwork?
We do not complete disability or FMLA paperwork during an initial visit. If ongoing care is established, we can discuss these needs on a case-by-case basis.
Clinical Services &
Treatment Approach
What conditions do you treat?
We treat a wide range of psychiatric concerns, including anxiety, depression, trauma, OCD, perfectionism, and chronic stress. If your needs fall outside our scope — such as active substance use, active eating disorders, or dementia — we’re happy to offer a referral to a trusted provider.
Do you provide couples or family therapy?
Your initial visit is a comprehensive evaluation. We’ll discuss your current symptoms, personal history, and goals for treatment. You’ll have time to ask questions and collaborate with your provider on a personalized care plan.
How long are sessions?
Initial psychiatric evaluations are typically 75–90 minutes, and therapy evaluations are about 60 minutes. Follow-up appointments vary but are generally 30–60 minutes depending on your treatment plan.
How often will I need to come in?
This depends on your needs and treatment plan. Some clients come weekly, others biweekly or monthly. We’ll work together to find a rhythm that supports your emotional well-being. Patients receiving medication management are seen on average 6-7 times in the first year and 4 times a year once stability is achieved.
Medication Management
What is medication management?
Medication management involves a comprehensive psychiatric evaluation, selection of appropriate medication, and the ongoing monitoring of response to psychiatric medications. At Gray MD Psychiatry, this includes assessing your symptoms, reviewing medical history, current prescriptions, discussing treatment options, and making collaborative decisions about whether medication is the right fit for your care.
Do I have to take medication to be a client?
Not at all. We believe in individualized care and respect each individual’s preferences. Medication is just one tool — and we’ll never pressure you to take a medication or engage in therapy. We’ll explore all of your options together.
How do I know if medication is right for me?
During your initial evaluation, we’ll take a deep look at your symptoms, history, and current life circumstances. If medication could help, we’ll explain your options, discuss risks and benefits, and decide together whether it’s the right step.
What if I’m already on medication?
If you’re currently taking psychiatric medications, we’ll review what’s working (and what’s not), consider any adjustments needed, and make sure your treatment is aligned with your current goals and health profile.
How often will I need medication follow-ups?
Follow-up appointments for medication are typically every 4–6 weeks, though this may vary based on your needs and how you're responding to treatment. These sessions are 30 minutes and include medication monitoring and supportive care.
Can I work with both the psychiatrist and the psychologist?
Yes — we often collaborate when medication and therapy are both part of your care. We’ll coordinate to make sure your treatment is seamless, personalized, and aligned with your goals.
Can I receive therapy and medication management in the same practice?
Yes! We offer both services in-house. You can work with Dr. Gray for psychiatric care and Dr. Mickens for therapy — allowing for more integrated, consistent support.
Therapy Services
What kind of therapy do you offer?
At Gray MD Psychiatry, we tailor therapy to each individual — there's no one-size-fits-all. Dr. Melody Mickens offers an integrative approach using evidence-based modalities like CBT, EMDR, and psychotherapy.
What is Cognitive Behavioral Therapy (CBT)?
CBT is an evidenced-based therapy that helps you recognize and change unhelpful thought patterns that impact your mood and behavior. It's especially effective for anxiety, depression, and perfectionism.
What is EMDR therapy?
EMDR (Eye Movement Desensitization and Reprocessing) is a research-backed therapy for trauma. It helps your brain reprocess distressing memories so they no longer feel overwhelming or disruptive.
How do I know what type of therapy is right for me?
You don’t have to know ahead of time — that’s our job! In your first session, we’ll explore your goals and symptoms, then choose a therapeutic approach that best supports your healing.
Can therapy really help me if I’ve tried it before?
Absolutely. We often work with clients who’ve tried therapy in the past but didn’t feel seen or supported. Our approach is personalized, respectful, and designed to help you reconnect with your inner strength and resilience.
